Dolby Digital Plus 4.0 (2/2 LCRS) Channel Mapping Issue

I'm having an issue. I get a file with the correct channel layout but incorrect content when I input a file with four channels and set 'coding_mode=5'. The left and right channels are fine. However, the center channel is empty and the back channel contains the center channel sound from the original file. I’ve tried all the 'use_channel_table options' but in each of it one channel is always empty. It seems that 'custom_channel_table' doesn’t work at all or I don’t understand the syntax for configuring it via the command line. As of now, I’ve worked around this issue using

But this seems like a strange workaround. How do I do this correctly?
